Sitagliptin + Dapagliflozin

Tablet

Sitagliptin is a DPP-4 inhibitor that enhances incretin activity, increasing insulin release and decreasing glucagon levels. Dapagliflozin is an SGLT2 inhibitor that lowers blood glucose by promoting urinary glucose excretion. Together, they provide complementary mechanisms for effective glycemic control, weight management, and cardiovascular protection in type 2 diabetes patients.

Mechanism

Sitagliptin inhibits DPP-4, prolonging incretin activity to stimulate insulin secretion and suppress glucagon. Dapagliflozin inhibits SGLT2 in renal tubules, reducing glucose reabsorption and increasing urinary glucose excretion. The dual mechanism provides synergistic glycemic control.

Pharmacokinetics

  • Sitagliptin Absorption: Rapid oral absorption
  • Sitagliptin Elimination: Primarily renal
  • Dapagliflozin Metabolism: Hepatic via CYP3A4
  • Dapagliflozin Half-life: 12–13 hours

Side Effects

  • Urinary tract infections
  • Genital mycotic infections
  • Nasopharyngitis
  • Hypoglycemia (with insulin or sulfonylureas)

Contraindication

  • Severe renal impairment
  • Hypersensitivity to Sitagliptin or Dapagliflozin

Interaction

  • Diuretics (risk of dehydration)
  • Insulin or sulfonylureas (risk of hypoglycemia)

Warning and Precautions

  • Monitor renal function regularly
  • Ensure adequate hydration
  • Use cautiously in elderly patients
